Important Notes: |
1. |
Age limit for the insured persons is between 1 to 79 years inclusive and renewable up to age 85 (Plan 1 and Plan 2), or between 1 to 70 years inclusive and renewable up to age 70 (Plan 3). Policyholder must be aged 18 years or above.

Child(ren) mean(s) child(ren) of the Policyholder who is/are unmarried, unemployed and between the age of 1 and 21 years or unmarried full-time student(s) between the age of 22 and 23 years.

If the insured person is below 17 or over 70 years of age, benefit under Accidental Death and Permanent Disablement is limited to 50% of the Maximum Benefit per Insured Person as stated in the Benefits Table.

Double indemnity (for Accident on Public Common Carrier): The amounts payable for death or permanent total disablement will be doubled to HK$600,000 (Plan 1); HK$1,200,000 (Plan 2) and HK$2,000,000 (Plan 3) if the accident occurs while the insured person rides solely as a passenger in or on, boarding or alighting from any public common carrier licensed to carry passengers for hire.

Double indemnity for Accident on Public Common Carrier is only applicable to insured person aged 17 years to 70 years at the time of the accident. |
6. |
Zurich Emergency Assistance: In the event of accidental injury or illness while the insured person travels abroad not exceeding 90 days, the insured person can call Zurich Emergency Assistance hotline for the following services:
a) |
Emergency medical evacuation and repatriation of the insured person back to Hong Kong.
|
b) |
Repatriation of mortal remains or local burial.
|
c) |
Hospital admission deposit guarantee of up to HKD39,000. The insured person shall reimburse to Zurich Insurance Company Ltd for any amount which is otherwise not covered under the plan.
|
d) |
Free return ticket for an immediate family member if the insured person is travelling alone and is hospitalized for more than 3 consecutive days overseas. |

Major Exclusions: |
This policy does not cover any loss directly or indirectly caused by, resulting from or in connection with any of the following: |
1. |
the time when the insured person is engaged in duty for the following occupations:
- |
any occupations principally in the use of light machinery or engines; or
|
- |
any extra hazardous occupations involving the use of heavy machinery, requiring high degree of physical exertion or working in an extra hazardous working environment; or
|
- |
engage in duty with any armed force of any country or international authority; fireman or
|
- |
stevedores, loading or unloading of objects on ships, being acrobats, aircrew, ship crew, aerial worker, racing driver, China-Hong Kong cross border driver, demolition workers, stuntman, entertainer, jockey, underground and underwater worker, explosive worker, construction worker, worker at height including but not limited to scaffolding worker, circus trainer, detective, newspaper-war correspondent, wild animal trainer; |
|
2. |
any pre-existing condition or related conditions;
|
3. |
any kind of disease or sickness; or any loss caused by an injury which is a consequence of any kind of disease or sickness;
|
4. |
any condition resulting from childbirth, miscarriage, abortion, pregnancy;
|
5. |
any cosmetic surgery, procurement or use of special braces, appliances or equipment except for it is reasonably caused by an accident;
|
6. |
the insured person’s participating in any illegal or unlawful acts;
|
7. |
riding or driving in any kind of motor racing, or engaging in a sport in a professional capacity or where the insured person would or could earn income or remuneration from engaging in such sport;
|
8. |
being a crew member or an operator of any air carrier;
|
9. |
any activity or involvement of the insured person in the air unless such insured person is at the relevant time (i) travelling as a fare-paying passenger on a regular scheduled flight or licensed chartered aircraft, or (ii) participating in such activity where the maneuver or navigation of such activity is responsible by another person who is adequately licensed for guiding such activity and the provider of such activity must be authorized by the relevant local authority;

10. |
trekking at an altitude greater than 5,000 meters above sea level or diving to a depth greater than 40 meters below sea level;
|
11. |
suicide, attempted suicide or intentional self-injury, insanity, mental disorder of any kind, psychosis, stress or depression, any condition under the influence of alcohol or drugs (other than those prescribed by a qualified medical practitioner), or Acute Mountain Sickness;
|
12. |
any injury, illness, death, loss, expense or other liability attributable to HIV (Human Immune Deficiency Virus) and/or HIV-related illness including AIDS and/or any mutant derivative or variations thereof however caused or however named;
|
13. |
war, invasion, act of foreign enemy, hostilities (whether war be declared or not), civil war, rebellion, revolution, insurrection, or military or usurped power, or direct participation in strike, riot or civil commotion or any kinds of participation in any act of terrorism; and/or
|
14. |
any expenses, consequential loss, legal liability or loss of or damage to any property directly or indirectly arising from:
- |
ionising radiation or contamination by radioactivity from any nuclear fuel or from any nuclear waste from the combustion of nuclear fuel; or
|
- |
the radioactive, toxic, explosive or other hazardous properties of any nuclear assembly or nuclear component.
|

Remarks:

- Fubon Bank is the licensed insurance agency appointed by Zurich , and is responsible for introduction of the insurance plans. The Plan is not the product of Fubon Bank. Fubon Bank is not responsible for approval, coverage and compensation under the Plan.
- In case you file a written complaint regarding the selling process or processing of the related transaction to Fubon Bank and the complaint is an “Eligible Dispute(s)” as defined in the Terms of Reference for the Financial Dispute Resolution Centre, Fubon Bank is required to enter into a Financial Dispute Resolution Scheme process with you if the Eligible Dispute cannot be resolved after Fubon Bank has issued the final written reply. If the complaint / dispute is related to the policy terms and conditions, it should be resolved directly between Zurich and you. Nevertheless, where practicable and appropriate, Fubon Bank will provide reasonable assistance to customers in the dispute resolution process.
- Levy collected by the Insurance Authority (if any) has been imposed on this policy at the applicable rate and would be remitted in accordance with the prescribed arrangements. For further information, please visit Zurich's company website: http://www.zurich.com.hk/ia-levy.
